We are seeking a dedicated Section 117 Mental Health Act Social Worker to join our Adult Social Care Mental Health team. This role is crucial for providing comprehensive care and support to individuals with mental health needs who are subject to Section 117 of the Mental Health Act. The successful candidate will be responsible for conducting reviews, ensuring timely care management, and developing person-centred support plans among other duties.
Day-to-Day Responsibilities:
Conduct Comprehensive Reviews: Undertake full reviews of individuals with mental health needs who are subject to Section 117 and eligible for social care support.
Ensure Timely and Ongoing Care Management: Maintain open status with Mental Health or Adult Social Care Services in Tameside to support continuity of care.
Review and Discharge from Section 117: Manage the discharge process for individuals no longer eligible for Section 117 aftercare in accordance with legal requirements.
Maintain Accurate Records: Ensure all documentation and social care packages are accurately recorded and uploaded to the electronic LAS system.
Support Timely Hospital Discharges: Collaborate to ensure prompt hospital discharges with appropriate aftercare arrangements.
Develop Person-Centred Support Plans: Co-produce support plans with individuals, their families, and other professionals to achieve agreed wellbeing outcomes.
Promote Independence and Community Integration: Support individuals transitioning back into the community by exploring flexible support options.
Review Eligibility and Reassess Needs: Regularly review Section 117 eligibility and reassess support as needed.
Support Multi-Disciplinary Working: Participate in Multi-Disciplinary Team meetings and maintain knowledge of relevant legislation and funding agreements.
Provide Expert Advice: Offer guidance on care placements and funding arrangements.
Foster Strong Partnerships: Maintain effective communication with Tameside Borough Council and Integrated Mental Health Services.
Required Skills & Qualifications:
Qualifications: Social worker qualification to degree level. SWE Registration.
Knowledge: Extensive knowledge of Mental Health and Social Care Legislation including the Care Act 2014, Mental Health Act, and Mental Capacity Act.
Experience: Post qualifying experience in mental health and adult social care practice. Experience in effective discharge planning from hospitals. Must have experience in S117.
Skills & Abilities: Effective risk assessment skills, ability to manage clinical situations, excellent communication skills, and computer literacy.
Work Related Circumstances: Current driving licence or supported driver with use of vehicle. Enhanced DBS check.
