Job Type: Hybrid (initially office-based for induction)
Working Hours: Flexible, between 8am and 7pm (standard day approx. 7 hours)
Join our dedicated team as a Neighbourhood Social Worker, where you will play a crucial role in providing comprehensive support and care within the community. This position involves working closely with multi-disciplinary teams to deliver a cohesive service to individuals in our neighbourhoods.
Day-to-day of the role:
Conduct strengths-based assessments under the Care Act 2014 and develop comprehensive support plans.
Support service users in accessing universal and voluntary services.
Perform carers assessments and formulate support plans in accordance with the Care Act 2014.
Adhere to the principles of the Mental Capacity Act 2005 and other relevant Social Work legislation and practice guidance.
Manage a busy duty system on a rotational basis, taking referrals, and providing information and advice at the point of contact.
Manage an active caseload, undertake safeguarding investigations, and participate in continuing health care assessments.
Attend multi-agency meetings and collaborate effectively with other professionals.
Required Skills & Qualifications:
Proven experience in carrying out assessments and formulating support plans under the Care Act 2014.
Experience in managing an active caseload and undertaking safeguarding investigations.
Familiarity with multi-disciplinary working environments.
Strong knowledge of the Mental Capacity Act 2005 and other relevant Social Work legislation.
Ability to work flexibly and travel within the Bolton area to meet service users in various settings.
