About the Role
We are seeking a compassionate and organised Care Co-ordinator to join an ambitious and growing service supporting children with complex, serious or life-limiting conditions. This is a rewarding opportunity to make a meaningful difference, working closely with children and their families both in hospital and at home.
You will play a vital role in coordinating care, supporting hospital discharges, and ensuring smooth transitions between clinical and community settings. Working as part of a collaborative multi-disciplinary network, you will help deliver high-quality, person-centred support to those who need it most.
Key Responsibilities
Manage a caseload of children and families, providing tailored support both in hospital and at home
Work closely with hospital discharge teams to enable safe and timely transitions from hospital to home
Act as a key liaison between hospital teams, community services, and families to ensure seamless care coordination
Support children and families emotionally and practically through complex and challenging situations
Maintain accurate records, reporting, and data management to a high standard
Build effective partnerships with healthcare professionals and stakeholders
Contribute to continuous service development and improvement

Essential Skills & Experience
Experience working with children and families with complex care needs in hospital and/or community settings
Strong communication and interpersonal skills, with the ability to build trust and rapport
Ability to work collaboratively with multidisciplinary teams
Excellent organisational skills, with the ability to manage competing priorities effectively
Confident using IT systems, including databases and reporting tools
Desirable
Experience in discharge planning or bed management
