Utilise the Manchester Tele-Triage System (MTS) for thorough assessment and follow-up with patients. Determine the urgency and criticality of member conditions to facilitate appropriate referrals. Coordinate with mental health services, emergency services, and other healthcare facilities as needed. Make necessary appointments on behalf of members and ensure continuous care by working with multidisciplinary teams. Provide referrals to counselling and partner services according to member needs. Carry out follow-up calls to evaluate the need for further assistance and support continuity of care. Collaborate with a team of doctors, nurses, and mental health professionals to provide the most appropriate and accessible outcomes for our patients. Required Skills & Qualifications Bachelor of Nursing/BSc(Hons) registered with the NMC board without restrictions. Band 6 experience with a strong background in accident and emergency. Mandatory experience in Triage and Emergency Department. Experience in Mental Health is advantageous. Strong team player with excellent communication and assessment skills.
