Location: Homeless Hostel Service Hours: 35 hours per week (Monday–Friday, rota shifts: 7:30am–3:30pm / 1pm–9pm, occasional cover required) Contract: Temp to Perm
About the Role
We are recruiting for a Case Management Worker to support individuals experiencing homelessness within a supported accommodation setting. You will work with vulnerable adults who may face challenges such as mental health issues, substance misuse, or involvement with the criminal justice system, helping them move towards independent living.
Key Responsibilities
Carry out resident inductions and support new arrivals
Develop and deliver person-centred support plans and risk assessments
Hold regular one-to-one sessions to review progress and set goals
Support residents with housing applications, benefits, and move-on plans
Maintain accurate records, reports, and case notes
Work closely with external agencies (e.g. social services, probation, healthcare professionals)
Promote resident engagement, independence, and wellbeing
Ensure safeguarding and health & safety procedures are followed
Conduct regular property and welfare checks
About You
Experience working in a support, housing, or similar environment
Understanding of homelessness and complex needs
Strong communication, organisation, and report writing skills
Ability to manage a caseload and prioritise workload effectively
Confident in risk assessment and support planning
IT literate (Microsoft Office and case management systems)
A compassionate, proactive and resilient approach
Desirable
Relevant qualification in Health & Social Care, Housing, or similar
Knowledge of welfare benefits and housing processes
This role is being recruited on a temporary to permanent basis.
