We are seeking a Care Manager to assist a team of experienced neurological case managers. This role primarily focuses on enhancing the quality of care provided to clients with neurological and/or complex traumatic injuries, while also overseeing aspects of HR such as recruitment and retention of care staff. The ideal candidate will be an integral team member, effectively collaborating with case managers and the HR department.

Day-to-Day Responsibilities:

  • Care Coordination:

    • Assist in the development and implementation of comprehensive care plans and risk assessments.
    • Regularly update client home files and ensure compliance with health and safety standards.
    • Monitor the effectiveness of care delivery and suggest improvements.
    • Support case managers in managing complex client cases and ensuring high standards of care.
    • Act as a liaison between clients, their families, and the care team to ensure effective communication and service delivery.
  • HR and Recruitment:

    • Lead the recruitment process for care staff, ensuring a high calibre of applicants through effective advertising and interviewing techniques.
    • Conduct induction sessions for new staff, focusing on high-quality care standards and client-specific needs.
    • Implement training programs that enhance the skills of care staff, particularly in areas related to neurological and complex care needs.
    • Oversee the maintenance of HR documentation, ensuring all processes are compliant with legal and regulatory standards.

Required Skills & Qualifications:

  • Proven experience in care coordination or a similar role within a healthcare setting, with a strong focus on neurological or complex care.
  • Excellent understanding of care planning, risk assessment, and patient confidentiality.
  • Strong recruitment and staff management skills, with the ability to identify and nurture talent in care provision.
  • Exceptional organisational and communication skills.
  • Knowledge of healthcare regulations, particularly those set by the Care Quality Commission.
  • Ability to handle sensitive information with high levels of confidentiality and compliance with GDPR.
